To order Prints fill out the Order Form below with the following information (In the Comments section):

• Players First and Last Name
• Team Name - Friday or Sunday
• Player Number​
• Forward or Defeceman 
• Cell number & email address if any questions need to be asked
• Please state if you would like Template or No Template for 4x6 & 5x7 Portrait Print
• Prints / Package / Items that you would like to order. Quantity number also ( 2 -4x6 /  1 In the Game / 8-Trading Cards etc)

Payment can be made during Sunday Photo's - March 5th . Please make cheques Payable to Susan Halverson
